Patient Information

NEW PATIENTS: We are a consultant practice. All new patients must be referred to us by your primary care physician. For patients who have appointments scheduled several months out, it is most helpful to have your records forwarded. After reviewing these records, you will be put on a waiting list for a sooner appointment. Please click the link to download new patient forms.

CURRENT PATIENTS: Please update your personal and insurance information via the patient portal (link to patient portal) or bring the information with you to your next visit. Remember to get your lab work done 2-4 weeks prior to your scheduled appointment.

Please use the patient portal or call the office during regular business hours for routine business, such as: to schedule or change appointments, check on lab results, request prescription refills, or speak to someone in the billing department. There is a provider on call 24 hours for an emergency only.  Refill request will be handled within 24-48 hours from the time requested.  Refills WILL NOT be done for patients who habitually no show/cancel appointments.

DIABETIC PATIENTS: Just a word about blood sugars; All insurance companies, including Medicare, require we submit blood sugar logs and office notes in order to obtain authorization for your testing supplies. The blood glucose record must be in your chart. Please be sure to bring your blood sugar readings to every office appointment.
Pump patients: Insurance companies, including Medicare, require that we send pump downloads every 3 months for Medicare and every 6 months for other insurance companies in order to obtain pump supplies.  Please be prepared and either download from home or allow us to download in the office.  Pump patients MUST be seen a minimum of every 3 months as per office policy and insurance guidelines.  Medicare mandates every 90 days and our office staff will accommodate this scheduling.

WE REQUIRE 24 HOUR NOTICE FOR CANCELLATIONS. If a scheduled appointment time has been set aside for you, that you are unable to keep, please notify the office as soon as possible and we will do our best to accommodate your needs. We reserve the right to charge a $50.00 fee for any cancellation without 24 hour notice.


"No Show" appointments and short notice cancellations lead to longer wait times for patients looking to schedule an appointment. Please be courteous and allow at minimum of 24 hour notice when at all possible.

"No Show" for a new patient consultation appointment will NOT be rescheduled. We set aside a large block of time for a new patient consultation that could be filled by another, should you have a need to cancel. Please be courteous to other patients in calling ahead if you are unable to keep your appointment.
